[The following real life report, containing no identifying personal information, was provided to Relph Benefit Advisors from the Nurse Advocacy program associated with our Lead²Health platform to demonstrate the impact of one life changed.]
47 year old male identified from the D2 explorer report. Very receptive to the call from his Care Coordinator and said he was struggling with:
- Uncontrolled diabetes
- Chronic pulmonary issues
- Chronic pain in both lower extremities
- Cardiac problems
Additional wellness measures identified:
- BMI > 30
- Long smoking history
On PHQ 9 depression screening he scored low positive.
His Care Coordinator determined that he is currently being treated by his doctor for the depression but suggested he talk with his doctor about how he scored on the screening. The Care Coordinator also reviewed his medications and adherence was good.
The Care Coordinator assessed for knowledge gaps and he was appropriately educated. In addition she discussed goal setting with member. Has desire to stop smoking in the next 6 months, lose weight and manage his diabetes.
He agreed to a referral to the health management program for the diabetes program and to programs for weight loss and smoking cessation.
In a subsequent call with this member, the Care Coordinator determined that he had started working with the smoking program and set a smoking quit date for January. He also followed through with his Primary Care Physician regarding his depression score and started on a new medication that is helping!
He has a start date for the HM program.
